The screening template is a very basic check to see if there is potential for a negative impact on people who share protected characteristics. Note that technically you don't have to include Marriage and Civil Partnership, but I have left it in for the sake of completeness.

Remember there is no statutory format for equality impact assessments. How they look is up to you, but keep your template short, avoid duplication and tell staff not to get too hung up on the boxes.
